The text is from 'A Crowne-Garland of Goulden Roses', London, 1612. This is the first printing of the Arthurian sonnet since that time. We have seen copies of the broadside issued as a broadside (not folded or in wrappers), but this was clearly issued by the printer/illustrator in this form.
